He, in his welcome note, the increase in Diabetes and Hypothyroidism is due to lifestyle related changes and genetic predisposition and such cases are on the rise in the region. He welcomed everyone present for the enlightening session. Dr. Binish Sreekumar, Specialist Internist, introduced Dr. Ramesh Gomez by throwing light on this rich academics and 15 years experience in the field of Endocrinology.

Presenting his first topic “Management of Hypothyroidism”, he mentioned Hypothyroidism is a condition where the thyroid gland does not produce sufficient hormones. Mostly it occurs as a result of autoimmunity. The guidelines of management of hypothyroidism keeps changing as the evidences from studies are accumulating. TSH cut off for different age group is different and there is a trend to over diagnose and over treat hypothyroidism. Pregnancy is a special situation where the clinician should decide whether their patient needs to get TSH checked and whether thyroid medications are to be prescribed. Patient needs to adjust the dose once they become pregnant and should get their TSH more frequently checked.

In his second topic “Management of Type 2 Diabetes”, Dr. Gomez said we should manage pre-diabetes (a wake-up call that you're on the path to diabetes) well to bring blood glucose level back into the normal range. Lifestyle modification (LSM) is the cornerstone of management in all stages of diabetes. Adequate sleep, stress free life, calorie restriction and physical activity need special emphasis. The target of glycemic (associated with carbohydrates) control needs to be individualized depending on the duration of disease, co-morbid illnesses and life expectancy of the individual. On a question asked related to Keto Diet by one doctor, Dr. Gomez said that it can be used as a short cut to lose weight, control diabetes, but should get their lipid profiles checked and cardiovascular risk well attended.

In his closing remarks Dr. Shafeek Mohammed, Group CEO said the common notion of considering endocrinology as only diabetes should be changed. Endocrinology covers a wide spectrum of diseases like Osteoporosis, PCOS, Thyroid Disorders, Growth Abnormalities, etc. He requested general physicians and internists that in case of uncontrolled diabetes and complicated cases, the patients should be referred to specialist endocrinologist. He emphasized on the importance of conducting such CME session and its contribution on the treatment outcome and patient wellbeing.
